The personal data owners (hereinafter referred to as "Applicant") defined as the concerned person in the Law No. 6698 on the Protection of Personal Data ("LPPD") are granted the right to make certain requests for the processing of personal data in Article 11 of the same law. Pursuant to the first paragraph of Article 13 of the Law on the Protection of Personal Data, the applications that will be made for such rights to our company acting in capacity of Data Controller, must be made in person and written in Turkish or must be delivered by other methods determined by the Personal Data Protection Board ("the Board").

Availability of below information is obligatory in the application as per Notification Regarding Application Principles and Procedures (dated 10/03/2018 and numbered 30356) for Data Controller.
Information to be filled in application:
• Name, surname and signature if it is a written application,
• TRIN for Turkish citizens, nation, passport number and identity number if any for foreigners,
• Settlement as base of the notification or work place address,
• Electronic mail address, telephone and facsimile number as base for the notification,
• The subject of request

In case that you want to make an application regarding your personal data, it is required to fill below form in full. This application form was issued in order to respond for your related application in correct and legal period by determining your relationship with our company and determining your personal data in full processed by our company. Our Company reserves the right to request additional documents and information (copy of the identity card or driver's license) for identification and authorization purposes to eliminate legal risks that may arise from data sharing in an illegal and unlawful manner, and particularly, to ensure the security of your personal data. In cases where the information on the request you submit under the form is not correct and up-to-date or an unauthorized application is made, our Company declines responsibility for the claims that may arise due to such incorrect information or unauthorized application.
As per paragraph 2 of article 13 of KVK Law, your application those are delivered to us, shall be responded free from charge in written at least within thirty day since it delivery date based on the nature of related request.
